To request completion of licensure-related documentation
including pharmacist-intern and pharmacist application
paperwork, please complete this form. Once your request has
been submitted, an email confirmation and/or request for
additional information will be sent to you via your Drake email
account.
It will take one to five days to complete your request, depending
on the volume of request received. Occasionally, turn-around
times may take longer due to holidays, vacation, etc. If you are
requesting notary services, you will receive a follow-up email
from Kristin Honold, Compliance Coordinator, with additional
information on setting up an appointment. |
